Associations of insulin resistance-related indices with the risk and progression of cardiometabolic multimorbidity in individuals with metabolic dysfunction-associated steatotic liver disease: a prospective cohort study

Research question

How are insulin-resistance-related indices associated with the risk and progression of cardiometabolic multimorbidity (CMM) among people with metabolic dysfunction-associated steatotic liver disease (MASLD)?

Study

This prospective analysis included 109,604 UK Biobank participants with MASLD who were free of cardiometabolic disease at baseline. Nine insulin-resistance-related indices were assessed using Cox, multistate, prediction, and exploratory mediation analyses.

Key finding

During a median 15.9-year follow-up, 4,944 participants developed CMM. All nine indices were associated with higher risk, with the strongest association for TyG-WHtR (highest versus lowest quartile: HR 2.70, 95% CI 2.45–2.97). The indices modestly improved prediction beyond conventional risk factors; inflammatory, hepatic, and renal biomarkers jointly explained 11–23% of the associations.
